Updated Forms Submission Process
We’ve made IRB submissions easier by creating fill-in forms for application documents.
Here’s What You Need to Do:
- Identify Your Application Type
- Visit the Types of Applications page to determine if your application is Exempt, Expedited, or requires a Full Review.
- Complete Appropriate Forms
- Fill out the application form that matches your application type.
- Optional but recommended: Use our fill-ins for additional documents (e.g., the “consent on behalf of a child” form). You may also create your own, but we still suggest consulting our templates to ensure you cover all essential information.
- Attach Documents in DocuSign
- Upload each completed document (e.g., research summary, informed consent) to the Signature Submission page in DocuSign.
- Set up the Signing Process
- List the Faculty Advisor as the Primary Advisor on the DocuSign Submission page.
- Include all researchers involved on the Submission page.
